[process-voms*] Remove duplicate entries on input

  • Actively check the setting of property voms.skip_ca_check,
  • Remove duplicate user entries that only differ in CA
  • Avoid attempting to create/handle duplicate VOMS accounts.

Tested adequately for deployment on voms2.
