The logcleaner script by default creates the output file in /tmp.
Another user with access to the machine can create a symbolic link with
the same name as the output file. This will make the user running the
logcleaner script overwrite another file on the system.

This patch fixes this by making sure that we don't open existing files.

The InfoCard modules stores files in /tmp, but does this in a way that
allows another user with local access to the machine to overwrite any
file the webserver has access to.

This patch fixes it by moving the files into the simpleSAMLphp temp
directory.

This fixes a potential situation where another user creates the
temporary before simpleSAMLphp attempts to create it. Anther user
creating the directory could make simpleSAMLphp overwrite arbitrary
files on the system.

Enable the assertion handler for all pages which load www/_include.php.
This may lead to more logging of assertion failures, but should
otherwise be invisible.

Instead of throwing an exception for all assertion failures, simply log
an error with a stack trace. PHP will still abort execution if
assert.bail is enabled.
