Add 'attributes.NameFormat' as the preferred name for this option, so
that it matches what the metadata parser extracts. Backwards
compatibility with 'AttributeNameFormat' is also provided.

We should probably update this library to the latest version, but that
must wait until we can test the changes. This should at least fix the
deprecated-function warnings that PHP produces.

Since pass-by-reference has been removed in PHP 5.4, it should no
longer be used. Both of these places probably used it by mistake,
so removing it should be safe.

It appears that there is some special processing of elements named "submit"
when triggering form-submit through javascript. This patch adds a workaround
for that.
