Move SimpleSAML_Utilities::generateID() to SimpleSAML_Utils_Random::generateID(). Deprecate the former and schedule it for removal in 2.0.

htmlentities uses the ISO-8859-1 charset by default, which breaks when
we are using UTF-8 data. It also emits entities that are invalid in XML.

cas.php is a frontend for login.php, validate.php and
serviceValidate.php. It allows them to be called as cas.php/login,
cas.php/validate and cas.php/serviceValidate and is meant for clients
like phpCAS which expects one configured prefix to which it appends
login, validate and serviceValidate.

The apache configurationchanges Dubravko Voncina mention is
thus not necessary any longer.

By default '$' matches end of string, or newline at end of string,
which made it possible to add a trailing newline to some strings that
shouldn't have it.

As far as I can tell, none of the errors presented a security problem.
