Old PHP templates are going to disappear anyway, so let's not break anything for a change that will be removed in the near future. We keep the .tpl.php extension though.

Fix Psalm-issues in the modules-directory

Also move the templates to the "saml" module, since they belong in there.

Don't start a <dl> when there's no content for it. Would result in "<dl></dl>".

One error left in: data-clipboard-target="#metadata"
JavaScript should probably be changed to check for id instead of a proprietary attribute.

Add mandatory cols-attribute for non-CSS compatible browsers.

htmlentities uses the ISO-8859-1 charset by default, which breaks when
we are using UTF-8 data. It also emits entities that are invalid in XML.

This patch changes the metadata parsing and generation to use
OrganizationName, OrganizationDisplayName and OrganizationURL.

It also updates users of 'name' to check 'OrganizationDisplayName'
if 'name' is unset.

Implementeed better error handling. Separate static function for fatal error in utitilies that present a error the user. This page to the user shows a error text that can be localized to several languages, the error page also shows debug text, and a way to submit errors to the administrators.
